A high-risk pregnancy is one in which the pregnant person or the developing baby has an increased chance of health complications before, during, or after delivery. High-risk pregnancies require specialized monitoring, management, and care from a maternal-fetal medicine (MFM) specialist — also known as a perinatologist — working in close collaboration with the obstetrics team. Conditions that may classify a pregnancy as high-risk include pre-existing maternal health conditions such as diabetes, hypertension, heart disease, kidney disease, autoimmune disorders, thyroid disease, and obesity. Pregnancy-specific complications including gestational diabetes, preeclampsia, placenta previa, placental abruption, intrauterine growth restriction (IUGR), preterm labor, and multiple gestation (twins, triplets) also place pregnancies in the high-risk category. Fetal factors contributing to high-risk status include chromosomal abnormalities detected through prenatal screening or diagnostic testing (such as amniocentesis or chorionic villus sampling), structural abnormalities identified on ultrasound, and conditions requiring fetal intervention. Advanced maternal age (35 years or older) is associated with increased risks of chromosomal abnormalities, cesarean delivery, and certain pregnancy complications. Management of high-risk pregnancies involves more frequent prenatal visits, enhanced fetal surveillance including serial ultrasounds (growth scans, anatomy surveys, and cervical length measurements), non-stress tests, biophysical profiles, and Doppler blood flow studies. For patients with preterm labor risk, interventions such as progesterone supplementation or cervical cerclage may be recommended. Conditions such as preeclampsia — characterized by high blood pressure and signs of organ damage, typically after 20 weeks of pregnancy — require close monitoring and may necessitate early delivery to protect both mother and baby.
